TERMS & CONDITIONS
Physical activity is inherently dangerous. I know and acknowledge that volleyball, like other physical activities, carries with it a risk of potentially serious injury and I am entering into this event with that understanding. I further agree that I will not enter and participate unless I am medically able, and by my signature (or that of my parent/guardian who has reviewed this if I should be under the age of 18 at the time of sign-up), I certify that I am able and willing to participate. I further recognize and agree that, should I be unable to continue participating in this event for any reason, the final decision to suspend my participation will be made by the appropriate event officials. I (or my parent/guardian) attest that I have read these rules and agree to abide by them. I (or my parent/guardian) further agree(s) that I will abide by the Center for Disease Control’s (CDC) recommendations for the prevention of the spread of COVID-19 and other communicable diseases, and I (or my parent/guardian) attest(s) to having read the CDC’s guidance at: https://www.cdc.gov/coronavirus/2019-ncov/prepare/prevention.html. I assume all such risks being known, appreciated, and accepted by me.
Having read this waiver and knowing these facts, I (or my parent/guardian), waive and release Points for Patriots Volleyball 2025, the town of Hempstead, Points for Patriots, Inc., all event sponsors, and their respective directors, representatives and successors from all claims or liabilities of any kind arising out of my participation in this event, even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver. Additionally, I (or my parent/guardian) grant(s) permission to Points for Patriots, Inc. to use my photographs, motion pictures, recordings or any other record associated with this event for any legitimate purposes. I understand that this event does not provide for refunds in the event of a cancellation, and by signing this waiver, I consent that I am not entitled to a refund if the event is cancelled before or during the event.
